How to use this form
Please take your time. First, the form will ask you to choose the total number of Sundays or Thursdays you wish to attend. Then it will ask you to choose the exact dates you want to attend.
Each date reads as “Thursday, June 17.” However, if you click on the drop-down menu, you will see all the Sundays and Thursdays available.
If your first meeting will be on Thursday, June 17, then choose that date. If your first time attending will be on a later date, choose Meeting #1, and use the drop-down menu to choose the date that corresponds with the first time you will attend. To add more dates, choose Meeting #2, and the date you wish to attend, and so on.
